Start with a general enquiry

Tell us your organisation, your role and how we can contact you. Please do not include detailed health information in an ordinary email.

We discuss the programme

We will explain current availability, eligibility, funding and whether the programme may be appropriate.

Confirm informed consent

The person being referred should know about and agree to the referral before identifiable information is shared.

Agree a safe next step

If suitable, we will explain the secure information and assessment process before programme participation.
